How to Apply:

Printout of the application form along with DD for Rs. 3,750/- (Rupees three thousand seven hundred fifty only) payable at Pune drawn on any Nationalized Bank in favour of “The Registrar, Dr. D. Y. Patil Vidyapeeth, Pune”, has to be sent by post or submitted in person to the Registrar's office, Dr. D. Y. Patil Vidyapeeth, Sant Tukaram Nagar, Pimpri, Pune – 411 018, Maharashtra, so as to reach on or before 19th December 2011 by 5.00 p.m.

• The candidate should submit photocopies of the following documents along with the application form:
• Statement of Marks of I, II, III, IV BDS examination
• Attempt Certificate
• BDS Degree/Passing Certificate
• Internship Completion Certificate
• Registration Certificate (DCI/State DC)
• Age and Nationality Certificate
• Bonafide Certificate from Dean
• Transfer Certificate
• Migration Certificate
• Proof of Date of Birth
• Two recent photographs, taken from same negative, without cap or goggles, taken within last six months. One should be pasted on the application form, duly attested by the Principal/Dean of the College or any Gazetted Officer. The other should be stapled to the application form.
